Tumor size remains the most consistent and reliable prognostic factor for node-negative patients. Infiltrating ductal carcinomas, not otherwise specified, or infiltrating lobular carcinomas measuring up to one centimeter in diameter have a 13% actuarial risk of relapse at 21 years, while special histologic types of node-negative cancer such as tubular, papillary, true medullary, mucinous, or adenoid cystic have a similarly low risk of recurrence with tumor diameters of up to three centimeters. Patients with tumors in these lowest-risk categories might gain no more than two percentage points in benefit distributed over ten years from systemic therapy.
